Empowering Families through Information

In the 2024/25 fiscal year, Sylvanvale held a series of six informative sessions aimed at empowering families, plan nominees and support coordinators with crucial information about impairments. Each session had between 8-10 attendees, with a total of 50 people joining both in person and via TEAMS.

The sessions were designed to provide detailed insights into what Sylvanvale could see in the NDIA PACE system regarding the listed diagnosis of the individuals they support. This information is crucial for families and support coordinators to understand and take necessary actions. Following the final session, the presentation and information were distributed via email to all plan nominees, ensuring that everyone, whether they attended or not, had access to this valuable information.

The response from the community was overwhelming. Sylvanvale’s Service Planning team was inundated with emails from parents and plan nominees who were eager to provide updated information or ask further questions. This surge in engagement highlighted the importance and impact of these sessions.

These sessions not only provided essential information but also fostered a sense of community and support among the attendees. The positive feedback and active participation from families and support coordinators underscore Sylvanvale’s commitment to transparency, communication, and empowerment.

By offering these sessions and distributing the information widely, Sylvanvale has taken significant steps to ensure that families and support coordinators are well-informed and equipped to make the best decisions for their loved ones.
