Supported Independent Living Handbook

Sylvanvale Supported Independent Living (SIL) is ideal for adults (18 years and over) wanting to live independently with access to 24/7 support in your own home.

This Handbook explains our model of support and outlines the important information we need from you to help us understand your individual needs.  This Handbook also sets out our polices, practices, procedures and additional information in relation to these services.  When you sign a Service Agreement with us you are also agreeing that the policies, practices and procedures set out in this Handbook form part of our Agreement.

This Handbook may change from time to time, and when it does, we will let you know.
